If we revise this notice you will receive a copy of the new notice in the mail. R. Bacon Enterprises, Incorporated will post a copy of our current notice in our offices in a prominent location at all times.
Complaints
If you believe your privacy rights have been violated, you may make your complaint to us and the Secretary of Health and Human Services. To file a complaint with us, contact our Privacy Officer at (812) 897-5660.
You will not be retaliated against for filing a complaint.
